For physicians who have access to CMH's DR Web Ambassador, please use the login you were provided to gain access to our images.

For those physicians who have not been provided with access, we will be happy to provide you with a login and password to view the images online. Download and complete our confidentiality agreement. Access will only be given to licensed referring physicians. You must provide your medical license number on the form.
