Appointments may be scheduled by calling our centralized scheduling center at 805-988-0616. You will be directed to the appropriate office for your examination.

For your convenience, we provide bilingual staff at all of our locations.

You may Download Patient Registration and Insurance Liability forms to fill out in advance of your appointment and fax to 805-604-1722 which will expedite your sign in process the day of your examination.
​
If you prefer, you may hand carry the completed form with you and give it to the receptionist on the day of your examination.
